BENEFITS SUMMARY

Health Insurance
Employees may enroll in one of several health plans after 60 days employment. Employees' premium is paid by the company. Dependent/family coverage is also available.
Dental Insurance
Basic and preventive dental insurance paid by company after 60 days employment. Major Restorative and family dental benefits available.
Life Insurance
Term group life insurance policy with face amount equal to annual salary is provided after 60 days employment. Optional employee and family coverage is available.
PayFlex
A Flexible Spending Account program (money set aside on a pre-tax basis for medical expenses not covered on health insurance or dependent care needs) is available after 60 days employment.
Serious Disability
Voluntary Serious Disability Insurance is available to employees on the first of the month following 60 days of employment.
401K Retirement
Employees over the age of 21 may sign up for a 401K retirement plan after one year employment. The Company will match $.25 on the dollar, up to 8% of the employees' gross income.
Leave and Holidays
New full time employees receive four sick days per year after 90 days employment and one week vacation after one year employment. Thereafter, vacation leave is increased dependent upon length of service. An equivalent of six paid holidays is also available after 60 days employment.
